This application is used to establish new water and/or sewer service with the Indian Valley Community Services District (IVCSD). Individuals requesting service must provide basic identification and contact information, including their name, date of birth, Social Security number, state-issued ID number, phone number, and email address. The application also requires the service address, mailing address (if different), the requested start date for service, the service classification (such as residential or commercial), and the location of service within the district (Greenville, Taylorsville, or Crescent Mills).
Applicants must indicate whether they are the property owner or a renter. If the applicant is renting the property, additional information about the property owner—including their name, address, and phone number—is required. Renters should note that property owners may be notified if an account is at risk of termination due to non-payment.
A $200 deposit is required to begin service. The application may be completed online or printed and submitted by mail or in person with payment by check, cash, or money order. Credit card payments may also be made over the phone or in person. By signing the application, the applicant agrees to pay for services in accordance with the rates, policies, and regulations established by the IVCSD Board of Directors.
Rebuilding Application.pdfResidents who lost their homes in the 2021 Dixie Fire must submit a rebuilding application to request water service, sewer service, or both from IVCSD. The application collects basic information needed to restore utility service, including contact details, the requested date to reestablish service, service classification, the type of service requested, the type of dwelling being rebuilt, and whether the property has an existing well.
To help coordinate the rebuilding process, the application also asks for the Assessor’s Parcel Number (APN) and contact information for construction-related professionals involved in the project, such as the architect or engineer, contractor, and fire suppression engineer.
Providing complete and accurate information helps IVCSD coordinate services and support a more efficient rebuilding process.
Discontinue Utility Service.pdfThis abandonment form is for existing customers of the Indian Valley Community Services District (IVCSD) who plan to demolish a structure or permanently discontinue utility service at their property. It allows property owners to request the abandonment of underground water service, sewer service, or both.
The form collects basic information about the property owner and the service location, including contact details, the site address, Assessor’s Parcel Number (APN), account number, and the type of building on the property. This information helps IVCSD identify the correct service connection and coordinate the requested work.
Property owners must also certify that they are authorized to request the abandonment of the services and that the work will not create a risk to public health, safety, or the property. The request includes acknowledgment that significant costs may apply if water or sewer service needs to be re-established in the future.
Submitting this form allows IVCSD to review the request, coordinate any necessary work to properly abandon the utility connections, and maintain accurate service records.
Note: Removal of a water meter does not indicate that it is safe to begin demolition activities.

Submitting the completed intake form through any of these methods will allow the District to process your public records request.
